Please bring to the school site:
1. Birth certificate or baptismal certificate or passport, etc.
2. Social security card for child (optional)
3. Proof or residence (address verification) - i.e., PGE bill, rental agreement. (Please see bottom of proof of residency form for further information. Three district- approved forms of proof are now required)
4. AN IMMUNIZATION RECORD (SHOT RECORD) MUST BE PRESENTED UPON REGISTRATION. Children will be registration in a class until immunizations are in compliance.not
The following immunizations are required:
A. TB skin test - PPD/Mantoux test given after 3/1/07
B. 3 Polio - last booster after 4th birthday
C. 4 DTP - last booster after 4th birthday
D. 3 Hepatitis B - 1st dose must be received before school entry
2nd dose 1 - 2 months after 1st dose
3rd dose 2 - 6 months after 2nd dose
E. 2 MMR (Measles/Mumps/Rubella) - after 1st birthday
F. Varicella (Chicken Pox) - must be received before school entry (or provide date of Chicken Pox infection).
Please do not wait for the immunizations to register. If you can prove #1 and #2 of the Registration, please come in between 9:00a.m. and 1:00 p.m. (or call for an appointment if these times do not fit into your schedule) and pickup a packet to register and you will be given some time to bring in the immunizations.

Kindergarten Registration
School tours are offered for prospective parents and guardians on TBD, Jan 2010, 1:00pm-1:45pm or TBD, Jan 2010, 9:00am-9:45am. Parents who are interested in a tour should meet in front of the office BEFORE the schedule tour start time. Please note that these tours are for adults only.

REGISTRATION FOR THE 2010 – 2011 SCHOOL YEAR:
Kindergarten Registration for 2010 incoming Kindergarten students and new students, in all grades, to Laurelwood, will begin in January, 2010. Parents are encouraged to register their incoming Kindergarten student during the month of January, between the hours of 9:00 a.m. to 1:00 p.m., all other times during the month of January by appointment only. Registration packets will be available in the Laurelwood office from 9:00 a.m. to 1:00 p.m. beginning January TBD, 2010 through February TBD, 2010.

SCUSD KINDERGARTEN PROGRAM: Beginning the Journey to Success
Program
The Santa Clara Unified School District implements a full day Kindergarten program. The Kindergarten schedule follows the current schedule for Grades 1 and 2. The Kindergarten teachers believe that a full day allows students to participate in a program that integrates the core curriculum with activities that are developmentally appropriate for children. It also provides more time to allow students to explore and experience music, drama, movement, art, science and social studies in a creative and relaxed environment. A flexible day allows for academic learning and quiet, restful times as needed. Class size in Kindergarten is 20 students, the same size as Grades 1-3.
Options
We recognize that some families may feel a full day Kindergarten program will not meet the needs of their child. Parents shall have the option of having their child attend the full day (the primary grade schedule) or a partial day (leaving just before lunch). Parents will have an opportunity to indicate their selection of the Full Day Option or Partial Day Option upon request. This form should be completed at the time of registration. Parents who wish to select the partial day option will be asked to meet with the teacher to clarify their selection and options.
Conditions
The full day Kindergarten program will have some impact on service. Students will receive an integrated curriculum throughout the day. Students who attend partial day will not receive instruction that occurs in the afternoon. Kindergarten students who remain for the full day will eat lunch at school and may buy or bring lunch.
Changing the Option
Parents will have an opportunity to change their option at the end of the trimesters. Parents who wish to change to the partial option will be asked to meet with the principal and/or teacher.
Commitment
Once parents have made a selection, it will be the parents'responsibility to ensure that the student is in attendance daily according to the option selected.
Background
The recommendation to implement a full day Kindergarten is the result of extensive study and discussion among all district Kindergarten teachers and elementary principals, district staff, and parents of Kindergarten students. The decision to implement a full day Kindergarten program also involved a thorough examination of relevant data from other states and other schools about the effects of full day Kindergarten on student performance.
